I Am Abel Community Health & Wellness is committed to addressing health disparities that disproportionately affect Black communities while providing free, accessible health and wellness services to all.
Our focus is on prevention, early detection, health education, and connecting individuals and families to ongoing care and community resources.

Community Health & Wellness Services
Our programs focus on some of the most pressing health concerns disproportionately affecting Black communities, including:
• High blood pressure and cardiovascular disease
• Diabetes and prediabetes
• High cholesterol
• Obesity, nutrition, and physical activity
• Kidney disease
• HIV and sexual health
• Cancer prevention and early detection
• Mental health and emotional wellness
Free community health screenings
include blood pressure, blood glucose, A1c, cholesterol, and rapid HIV testing.
In addition to screenings, I Am Abel Community Health & Wellness provides health education, wellness programming, community health events, and connections to primary care and other community resources.
Who We Serve
Our services are free and open to all. We place a special emphasis on addressing health disparities and barriers to preventive care that disproportionately affect Black communities and other historically underserved populations.

I Am Abel Community Health & Wellness provides preventive health screenings, health education, wellness programming, and connections to community healthcare resources. Screening services are not a substitute for comprehensive medical care, diagnosis, or treatment by your healthcare provider.
Individuals with abnormal screening results will be encouraged to follow up with an appropriate healthcare provider and may receive assistance connecting with primary care and other community resources.
